Medical exams are mandatory by the United States Citizenship and Immigration Services (USCIS) before you are approved for a Green Card. Only doctors designated by USCIS as Civil Surgeons can complete an immigration physical. Our Civil Surgeons have successfully performed thousands of these exams, followed strict guidelines established by the DSH, USCIS, and the Centers for Disease Control (CDC) to satisfy immigration exam requirements for patients of all ages.

I-693: Immigration Physical
Your Physical Exam will include at least 2 visits. Your First visit will allow us to review your paperwork and order the required tests (consultation). Your second visit involves being seen by our Civil Surgeon who will review the laboratory results and conduct a physical exam. The Civil Surgeon will also ensure that you have satisfied all requirements of the I-693 process.

(Add $50 per applicant, if pay with Credit Card / Debit Card due to processing fees).

This fee covers for 3-5 visits, as needed to obtain all necessary medical history for any patients with medical conditions that prevent them from studying for the English Naturalization Exam and Civic Materials.

The immigration medical examination entails a review of your medical history and a physical examination.

The Civil Surgeon will complete a comprehensive examination that will include a review of your medical history and a physical examination. The Civil Surgeon will also test for communicable diseases such as Tuberculosis, Syphilis, and Gonorrhea, depending on age, as required by the Centers for Disease Control and Prevention (CDC).

After the medical examination, the Civil Surgeon will complete Form I-693 and seal the form in an envelope for you to submit to USCIS. Make sure you get a copy of the completed I-693 form for your personal records before the Civil Surgeon seals the envelope. USCIS will not accept the form if the envelope has been opened or altered.
Some health conditions require a medical waiver, like severe allergic reactions to certain vaccines, active tuberculosis, untreated syphilis or gonorrhea. If found, your exam forms can’t be completed until treatment or resolved. But don’t worry – our experienced civil surgeons who are also family medicine doctors can help you apply for an immigration medical waiver. We’ll compile the needed medical records, start treatment, or referral if required. With proper treatment and paperwork, many applicants still get green card approval. Our clinic has a high success rate, so let us guide you through the green card process and please be open about your medical history. We are here to help you through this process !
What If You Require an Immigration Medical Waiver?

For patients who live out of town and would like to combine all in 1 visit, we require $150 deposit for tests to be ordered near you. Then, once results are available, will schedule for you to see the civil surgeon and pay the remaining balance in the same visit.
